DISCLOSURE TO OBTAIN BACKGROUND INFORMATION
As you know, you have applied for employment with Clinical Research Management
in a position that may involve the performance of services for the United
States Government. In order to be hired into one of these positions with Clinical
Research Management,
your application must be approved by the U.S. Government. This notice is to
inform you that, as part of the U.S. Government’s procedure
for considering you as a candidate for this position with Clinical Research Management,
or, in the event that you are employed in this position, at any time while
you are employed, the U.S. Government may obtain from a credit reporting agency
a consumer report or an
investigative consumer report containing financial and other information about
you. The U.S. Government may obtain one or more consumer reports regarding
you from one or more consumer reporting agencies and may use the information
provided in the report(s) for the purpose
of evaluating you for employment, promotion, reassignment or retention as an
employee in a position performing services for the government. A “consumer
report” is a communication of information by a consumer reporting agency bearing
on a consumer’s credit
worthiness, credit standing, credit capacity, character, general reputation,
personal characteristics, or mode of living. An
“investigative consumer report” may include information as to your character,
general reputation, personal characteristics and mode of living, and this information
may be obtained through personal interviews with your neighbors, friends or associates
or from others with
whom you are acquainted or who may have knowledge concerning such items of information.
This conduct may implicate the Fair Credit Reporting Act (or applicable state law), which provide prospective or current employees
with rights regarding consumer reports and place specific obligations on employers who obtain such reports.
In connection with this application for a position with Clinical Research Management, Clinical Research Management will not forward your
application to the U.S. Government (and thus the U.S. Government will not obtain either a consumer report or an investigative consumer
report), without your signed authorization.
